El descenso de la fecundidad, precedido por la baja de la mortalidad, se ha traducido en un cambio en la estructura por edad de la población, caracterizado por el envejecimiento. A la transición demográfica se suma la transición epidemiológica, definida a partir de los cambios en las causas de muerte y en la estructura por edad de las defunciones. Subsisten diferencias marcadas en las tasas de mortalidad infantil, en el marco de las inequidades sociales y el acceso a los servicios de salud. La fecundidad tiende a concentrarse en las edades intermedias, de menor riesgo reproductivo, aunque en algunos países la fecundidad adolescente es elevada.
